For Department of Veterans’ Affairs clients
Our services are available to eligible DVA clients holding a Veteran Card. If you are not sure which card applies, give us a call — we will help you work it out.

Veteran Card — All Conditions
Covers treatment for all clinically necessary health care needs, whether or not they relate to service. Community nursing is available with an approved referral.
- White Card
Veteran Card — Specific Conditions
Covers treatment for accepted service-related conditions. Community nursing is available where the need relates to an accepted condition.

How a referral works
Community nursing through DVA starts with a referral from a health professional. Here is the whole path, start to finish.

A health professional completes the referral
The Community Nursing Referral (Form D9389) must be completed by a GP, a treating medical practitioner in hospital, a hospital discharge planner, or a nurse practitioner specialising in community nursing.

The referral tells us what’s needed and where
Two details matter most: the location where the support will be provided, and a clear description of the support needed. The more specific the clinical picture, the faster we can match the right nurse.

We make contact and get started
We confirm eligibility with DVA, call to arrange a first visit at a time that suits, and agree a care plan with you and your family.
